Incident Reporting
The recording of adverse events, near misses and unsafe conditions, so patterns can be seen and acted on rather than absorbed.

An incident report captures something that went wrong or nearly did — a medication error, a fall, a mislabelled sample, equipment that failed. It records what happened, when, and in what circumstances. Mature systems record near misses as deliberately as actual harm, on the reasoning that the near miss is the same failure with better luck.
Why it matters in a hospital
The value is entirely in the pattern, not the individual report, and the pattern only appears if reporting is safe. A system used to attribute blame stops receiving reports within weeks, and the hospital loses sight of its own risks while believing they have reduced. The other common failure is reports that go in and nothing visibly comes out — staff stop reporting when nothing ever changes, which is harder to detect because the register still looks maintained.
How Trisvio handles it
Trisvio has no incident-reporting module. What it does hold is the factual record an investigation needs — who administered which dose at what time, who verified a result, what a bill was changed to and by whom — all audited, so a reconstruction rests on records rather than recollection.
