In short

The statutory record every genetic clinic or ultrasound centre in India must complete for each pregnant woman examined.

Digital health & compliance

Form F is the statutory record required under the PC&PNDT Rules, which every genetic clinic, genetic laboratory, genetic counselling centre or ultrasound/imaging centre must complete for every pregnant woman undergoing an ultrasonography or related procedure — capturing details such as the referring doctor, the medical indication for the scan, and a declaration that the sex of the foetus was not disclosed to anyone.

Why it matters in a hospital

A hospital or diagnostic centre offering obstetric ultrasound is legally required to complete and retain a Form F for every applicable scan, without exception — it is not left to clinical discretion. It is one of the specific records health authorities look for during a PC&PNDT inspection, and an incomplete or missing form is treated as a compliance failure in its own right, independent of what the scan actually found or was used for. Because the requirement applies per scan rather than per patient, a facility performing a high volume of obstetric ultrasounds accumulates a correspondingly large number of Form F records, which makes a reliable, searchable way to complete and store them a real operational need rather than an occasional task.

How Trisvio handles it

Trisvio's PC&PNDT register captures the Form F fields against the patient encounter at the point of the scan, locks the record once it's finalised so the entry can't be altered later, and rolls completed forms up into a monthly export formatted for statutory filing with the appropriate authority.
