Medical Certification of Cause of Death (MCCD) is the standard, internationally aligned format Indian hospitals use to record a patient's immediate and underlying causes of death. The treating or certifying doctor completes Form 4, used for most deaths occurring under medical attention, or the related Form 4A, and the completed certificate feeds into India's civil registration system alongside the formal death registration.
Why it matters in a hospital
When a patient dies in hospital, an accurate, promptly completed cause-of-death certificate is both a legal registration requirement and the basis on which the family completes the death registration and any subsequent formalities that depend on it. It is also how national cause-of-death statistics are compiled at scale, so an incomplete or delayed MCCD has consequences that reach well beyond the individual case in front of the certifying doctor. Consistent, complete MCCD data across many hospitals is part of how public-health authorities understand disease burden and causes of death at a population level, well beyond the immediate purpose it serves for any one family.
How Trisvio handles it
Trisvio maintains a death register with an MCCD Form 4 workflow, capturing the required certification details against the patient record at the time of death, in a format aligned with what civil registration filing expects, and rolling entries up for export. Keeping it as its own register, tied to the discharge and inpatient record, means the certifying doctor is working from the same documented diagnosis and treatment history rather than starting the certificate from a blank form.
